Synchrony’s Rehab Solutions
For over 25 years, Synchrony Rehabilitation has offered superior rehabilitation services to individuals in long-term care, assisted living and independent living, home health and outpatient settings. We were great at what we did, but we couldn’t help but ask ourselves, “Can we do more?” This led to the development of our full spectrum of Clinical Programming. These services focus on specific diagnoses such as Parkinson’s disease, stroke, pulmonary illnesses, and so many more. And of course, these programs can be customized based on the people we are serving and the tools we have on hand.

Synchrony offers a full service pharmacy with packaging options to fit your needs. Our team of consultant pharmacists and nurses includes recognized clinical experts and our dedicated tech support is there for you when you need them.
Synchrony Pharmacy also provides comprehensive reporting and data analytics to assist you in making informed decisions. We are committed to clinical excellence and actively engage in Antibiotic Stewardship, Vaccine Clinics, Falls prevention and actively involved actively involved in the Drive to Deprescribe.
Synchrony Lab
Synchrony’s full service lab provides state-of-the-art laboratory services for Long Term Care and Senior Housing. When combined with our Pharmacy & Therapy services, you’ll have the insights to make actionable decisions for better patient outcomes. Our in-house, full-service lab provides routine testing, leading to less STAT labs and more cost savings. Lab draws are available 6 days a week with results in 12-18 hours.

Quick Fact: In the US, 23% of people are re-hospitalized within 30 days after discharge. Only 4% of Rx@HOME users are.
How the Rx@Home Program Works
When you’re discharged
- You’ll go home with a 30-day supply of your medications in easy-to-use PackEDGE packaging.
- No money is needed up front for your medications.
- SynchronyRx@HOME clinical staff will contact you within 5-7 days to see how you are doing.
After 30 days
- Your medications can continue to be provided to you at home. You’ll receive your medications through the mail with automatic refills as directed by your doctor, in the same convenient packaging.
